The support from SCCCC is all aimed to provide older people with a helping hand when they need it most.
Our work helps older people in their own homes and we can also support them if they are admitted to or discharged from hospital or living in a residential home.

Good Neighbour Scheme

Remember, there are no strangers here, just friends you haven’t met yet.
The SCCCC Good Neighbour Scheme is our befriending service for the over 65s in Sheffield and Doncaster. Are you over 65 and live in Sheffield or Doncaster?

Perhaps you spend more time at home than you used to, or you feel isolated? Would you enjoy a regular visit from one of our dedicated and friendly visitors for a cuppa and a chat?

Don’t just take our word for it, but this SCCCC service could make all the difference, especially for older people on their own with no family nearby.

Good Neighbour Scheme

Telephone Support Service, Sheffield

Having someone to talk to regularly makes all the difference to our mental wellbeing.

Are you over 55, feel lonely or isolated, and would like to talk to someone over the telephone on a regular basis? As part of SCCCC services, we offer telephone support for older people living in Sheffield.

We match you with a friendly volunteer who rings regularly, making sure you feel socially connected. You never know, you may enjoy putting the world to rights and become good friends! Telephone Support Service Sheffield referrals can be from health professionals, family, friends, and potential service users.

PenPal Scheme, Sheffield

There is nothing nicer than receiving a handwritten letter through the post.

The SCCCC Pen pal Scheme was created in 2020 as a response to Covid-19. We were conscious of the need to keep vulnerable, isolated, and older people in Sheffield connected. So we invited members of the public to send us ‘happy post’ that we could pass on to service users of the Good Neighbour Scheme, at a time when face-to-face interaction was either banned or difficult because of social distancing. That’s how this SCCCC service began, and the Pen pal Scheme is still going strong!

Referrals to the scheme can be from healthcare professionals, family, or friends, or you can refer yourself.

Hospital to Home Service, Sheffield

Working closely with Sheffield Teaching Hospitals, ICB, and community health professionals, SCCCC services provide timely support to vulnerable and frail patients over 55 upon discharge from hospital. We will make sure patients have transport from hospital to home.

Not only that, but we will also be on-hand for emergency food shops, mobility equipment deliveries, access, and environmental visits.

Discharge Lounge Sheffield

The SCCCC Discharge Lounge provides assistance to those over 55 who have been in hospital and are well enough to go home with some support but have no other way to get home.

This SCCCC service ensures the patient is escorted home safely if they have no other way of returning home after a stay in hospital. Our dedicated team will make sure that patients are settled in happily with a cup of tea, and we check that the heating is on, if required.

Operating between 9 am and 5 pm from Monday to Friday, referral to the SCCCC Discharge Lounge is by health professionals only, who can call:

Trusted Assessor Scheme

What is it?

The Trusted Assessor Scheme is a national initiative designed to reduce delays when discharging people from hospital.

Providers adopt assessments carried out by suitably qualified ‘Trusted Assessors,’ who work under a formal written agreement. This SCCCC service is provided in Sheffield, assessing individuals in their home for minor aids such as toilet equipment or raising furniture. We also work proactively to help to prevent falls in the home, working with our service users to avoid trip and fall hazards, which may require cleaning floor spaces to enable clearer access around the home.

Trusted Assessor

Home from Hospital, Doncaster

Available to anyone over 18 living in Doncaster, the SCCCC Home from Hospital service is a person-centred plan beginning with pre-hospital discharge, then continuing for four weeks after discharge. We help patients to regain confidence following a stay in hospital, empowering them to reconnect with friends and neighbours in their local community.

We provide support with tasks like:

  • Cleaning/light housework
  • Assistance with funding applications
  • Sorting out bills
  • Shopping

Back Home Scheme, Sheffield

Similar to the scheme in Doncaster, Back Home Scheme Sheffield is a four-week person-centred plan to empower those over the age of 55 to settle back into their home and community and regain confidence following a hospital discharge. As well as offering support, Back Home Scheme Sheffield is a preventative programme to help reduce hospital readmissions.

Here are just a few examples of how we can help:

  • Support for claiming attendance allowance to help pay for long- term service requirements
  • Blue badge application support
  • Cleaning/light housework
  • Short-term help with sorting bills and setting up direct debits for longer-term needs.

  • Cleaning Light housework. Support to arrange deep cleaning or de-cluttering. Support to arrange long-term cleaning solutions
  • Laundry Helping to wash and dry clothing and bedding, helping to set up future laundry service if needed.
  • Shopping and if where mobility is good, going with service user to build confidence. Help to set up online shopping if needed or to locate a support worker for long-term needs
  • Food we will work with the service user to discuss meal options that suit their mobility and dietary requirements, such as ready meals or meals-on-wheels assessment for property external and internal (including level 1 falls risk)
  • Bills Help with paying bills in the short term, helping to set up direct debits for longer-term needs
  • Prescriptions Collection of prescriptions for the short term. Support to find a delivery service for the long-term if needed.
  • Medication prompting Help to find longer-term solutions such as Nomads or electronic prompting and dispensing machines.
  • Appointments Help to make medical appointments, escorting to appointments (mobility dependent) Help to find a long-term solution for accessing appointments
  • Support to claim attendance allowance, helping pay for services needed long-term
  • Link with community policing and other core services if the service user has concerns around safety.
  • Support to apply for a blue badge

Adverse Weather Support

Bad weather can have a serious impact on providing care within the community, causing delays and cancellations of important health appointments. Launched in 2022, SCCCC Adverse Weather Support assists health professionals to carry out health appointments across Sheffield during adverse weather conditions, when they cannot attend by normal means.

SCCCC staff have been trained to drive our specially adapted 4 x 4 all-terrain vehicle, allowing the safe transportation of staff and patients in all weathers.

Adverse weather

Temporary Key Safe Loan

Available for the over 55s, we loan out temporary key-safes for a period of 4-6 weeks to people who need them.

They help to prevent delayed hospital discharges by providing temporary key access for carers, until a permanent key-safe can be fitted.

High Seat Chair Loan

This SCCCC service involves the loan of high seat chairs for a period of 6-8 weeks.

These are primarily for patients who have had hip, knee, or heart surgery. Delivered and collected by SCCCC staff, chairs are loaned out on a first-come, first-served basis, providing patients meet the required criteria. We simply require a deposit of £20 per chair, which will be refunded on its return. In addition, we have a small number of end-of-life chairs available.

Referrals are accepted from health professionals, family, friends or potential service users.
