Health and social care

SCCCC supports vulnerable older people across Sheffield and Doncaster, helping them stay independent and return home from hospital sooner. Through services such as Back Home and Home from Hospital, our dedicated staff and volunteers provide practical and emotional support when it is needed most.

Working closely with health and social care partners, we deliver measurable impact, excellent value for money, and strong social return on investment.

We believe that only by working together can we make a real difference to the lives of older people, their carers’, friends, and family.

We enjoy a unique position in the cities through a working partnership with organisations such as Sheffield City Council, Sheffield Teaching Hospitals NHS Trust, Doncaster Royal Infirmary, St Luke’s Hospice and SSAFA.
